Better Care at Lower Cost for Low-Income Patients – Learn How Camden, New Jersey is Leading the Way

The New Yorker’s Atul Gawande and others are lauding an innovative care delivery model for low-income patients. This new Medicaid Accountable Care Organization (ACO) model, operating in Camden, New Jersey, is improving care, lowering costs and engaging patients and their families.

It is led by health care innovator Dr. Jeffrey Brenner, Founder and Executive Director of the Camden Coalition of Healthcare Providers. He will speak at a forum organized by the Campaign for Better Care this Friday, March 11. Brenner will be joined by: Joe Fleming, Director, PICO New Jersey; Ev Liebman, Outreach and Advocacy Director, New Jersey Citizen Action; and Jennifer Coleman, Senior Advisor, Centers for Medicare and Medicaid Services (CMS) Federal Coordinated Health Care Office. Speakers will share experiences and discuss lessons learned from building models that deliver better care at lower cost for highest need patients. The event will be:

The Campaign for Better Care aims to ensure that the reformed health care system provides the comprehensive, coordinated, patient- and family-centered care that older adults and individuals with multiple health problems need. It is advocating for better ways of delivering care including effective care coordination, transition management, medication reconciliation, support for patients and their family caregivers, and care that is culturally and linguistically appropriate.
