On Monday, June 6 at noon, Dr. Don Berwick — the head of the agency responsible for Medicare, Medicaid, and the Children’s Health Insurance Program, which together provide care to nearly one in three Americans — will be in Atlanta to talk with patients, consumer advocates and health care providers about a new initiative to promote safer care by reducing hospital acquired infections and unnecessary hospital readmissions. The Campaign for Better Care aims to ensure that the reformed health care system provides the comprehensive, coordinated, patient- and family-centered care that older adults and individuals with multiple health problems need. It is advocating for better ways of delivering care including effective care coordination, transition management, medication reconciliation, support for patients and their family caregivers, and care that is culturally and linguistically appropriate.
